I spent 7 years in the gym walking one hour and working out. I lost 85 pounds in about 2 years with a lot of hard work and dedication. After I left the gym in 2008 due to it closing I started having problems about a year later. I recently had surgery on my left shoulder on Jan 4th of this year to shave down the bone to leave enough space for my shoulder joint to work. What's Tarsal Tunnel Syndrome?

Tarsal Tunnel Syndrome (TTS) is a name for a group of problems that includes swelling, pain, tingling and loss of strength in your foot and ankle. This condition will happen when the tibial nerve in your ankle (which runs through the tarsal tunnel) becomes squeezed (compressed) by damaged tissue or bone inside or outside of the tarsal tunnel. This condition is similar to Carpal Tunnel Syndrome that happens in the wrist, but it's a lot less common than CTS.

What is the
'Tarsal Tunnel'?

The tarsal tunnel is found on the inside of your ankle and is formed in a similar way to the carpal tunnel. There are 3 major bones in your foot and ankle form the bottom of the tunnel (inside your ankle). This includes the calcaneus (heel bone), talus (a bone in your ankle) and tibia (shin bone). A thick band of fibrous tissuecalled the flexor retinaculum is the top of this tunnel and acts like a bridge over the tendons that run through this tunnel. The flexor retinaculum surrounds the muscles and nerves in your lower leg and ankle to protect them. Your ankle and foot tendons, muscles, nerve, artery and vein pass through the Tarsal Tunnel to get to the bottom of your foot.

One of the more major nerves that pass through the tarsal tunnel is the tibial nerve. This major nerve is what provides feeling to the bottom of your foot. Tarsal tunnel syndrome is a condition that you get when this nerve is squeezed by surrounding bone, muscle, tendons or ligaments.

What Causes Tarsal Tunnel Syndrome?

The tibial nerve in the ankle/foot can be squeezed in a few different ways:

  • By swelling caused by an ankle sprain,
  • Any soft tissue swelling inside or outside of the tunnel caused by other injuries - like a ganglion cyst, swollen tendon or bone spur,
  • Degeneration (wearing away of bone and/or tissue) in the ankle joint from conditions like arthritis,
  • Any nerve damage that's already existing in your ankle,
  • Or even anatomical, natural shape and size of your tarsal tunnel or other naturally occuring factors like if you have flat feel or fallen arches.

Tendonitis (sometimes spelled tendinitis and pronounced 'tendinitus') is a condition where your tendon is irritated or inflammed. Usually this irritation, swelling and inflammation happens because of some kind of damage or micro-tearing to the tendon. This can happen from overuse of the tendon or an accident of some kind.

Foot and ankle tendonitis (posterior tibial tendonitis) can cause tarsal tunnel syndrome, especially if the tendon affected runs inside or outside of the tarsal tunnel. What's the Difference Between Tendonitis, Tendinosis and Tenosynovitis?

Tendinosis is basically like tendonitis but it's not caused by an injury. Tendinosis is caused by chronic degeneration (wearing away) of tendon fibers over time. This is a natural thing that happens to our tissue as we age.

Where tendonitis is damage or micro-tearing to your tendon, tenosynovitis is damage or micro-tearing to the sheath covering your tendon. This sheath is a protective lining for your tendon and is made up of synovium. If this sheath is injured it can also swell up to take up space in your carpal tunnel and squeeze on the median nerve.

It's possible for you to have tendonitis / tendinosis AND tenosynovitis at the same time creating a more complicated wrist injury.

What is a Sprained Ankle?
How Does a Sprained Ankle Relate to TTS?

A sprained ankle is a stretch and/or tear that's happened to 1 or more of the ligaments in your ankle. You might end up with a sprained ankle if your foot twists, rolls or turns inward beyond its normal range of motion. You can easily sprain your ankle if your foot is planted unevenly on a surface.

Approximately 25,000 people suffer from the pain of a sprained ankle every day (reference: 1). There are 7 different kinds of ankle sprains that you can get including a high ankle sprain, lateral sprain, invesion sprain, eversion sprain, syndesmosis sprain, anterior talofibular ligament sprain or even an anterior inferior tibiofibular ligament sprain.

Swelling and inflammation from an ankle sprain can also cause tarsal tunnel syndrome by compressing the tibial nerve in the tarsal tunnel. Relief of swelling and inflammation through cold compression and conservative treatment will release the pressure on the tibial nerve.

How Severe is Your Sprained Ankle Injury?

Sprains are usually categorized according to severity:

  • MILD (Grade 1) - 1 or more ligaments in your ankle are stretched in a mild sprain, but there is no joint loosening.
  • MODERATE (Grade 2) - 1 or more ligaments in your ankle are partially torn, producing joint instability and some swelling.
  • SEVERE (Grade 3) - 1 or more ligaments in your ankle are torn completely, separated from the bone and causing you excruciating pain. This is a complete rupture of the ligament(s) and makes your ankle joint nonfunctional.

It's also possible to rupture a tendon in your ankle due to tendonitis or a traumatic accident.

You should seek medical attention if you suspect that you've ruptured a tendon or ligament in your ankle, can't move the affected joint and have numbness in any part of your ankle.

Over the past eight months I have been suffering from shoulder and arm pain. I just kept thinking that the pain would go away on its own. I can now raise my arm to just about shoulder height unassisted, and can now almost get my finger tips back behind my back. I make a point of using Freeze Wrap as soon as I get home from work to relieve the stress to my muscles from work and to keep the inflammation down. I am a 44 year-old woman in good health. I eat a fresh diet and take high-quality supplements, including MSM, glucosamine, Zyflamend, and all the usual joint-recomended supplements. I injured both knees two years ago while kneeling to tile my family room and entry. Subsequently, a simple twisting squat ripped my left meniscus with a huge bucket tear that required surgery to trim so it wouldn't keep catching and re-tearing. I am not one to undergo surgery unless it is unavoidable, and even after a "simple" scope operation, it took me a full year to get full extension back on that knee, and I doubt I will ever be able to squat again. I recently went to a Rheumatologist and was diagnosed with DeQuervains tenosynovitis.
