What's Tarsal Tunnel Syndrome?

Tarsal Tunnel Syndrome (TTS) is a name for a group of problems that includes swelling, pain, tingling and loss of strength in your foot and ankle. This condition will happen when the tibial nerve in your ankle (which runs through the tarsal tunnel) becomes squeezed (compressed) by damaged tissue or bone inside or outside of the tarsal tunnel. This condition is similar to Carpal Tunnel Syndrome that happens in the wrist, but it's a lot less common than CTS.

What is the
'Tarsal Tunnel'?

The tarsal tunnel is found on the inside of your ankle and is formed in a similar way to the carpal tunnel. There are 3 major bones in your foot and ankle form the bottom of the tunnel (inside your ankle). This includes the calcaneus (heel bone), talus (a bone in your ankle) and tibia (shin bone). A thick band of fibrous tissuecalled the flexor retinaculum is the top of this tunnel and acts like a bridge over the tendons that run through this tunnel. The flexor retinaculum surrounds the muscles and nerves in your lower leg and ankle to protect them. Your ankle and foot tendons, muscles, nerve, artery and vein pass through the Tarsal Tunnel to get to the bottom of your foot.

One of the more major nerves that pass through the tarsal tunnel is the tibial nerve. This major nerve is what provides feeling to the bottom of your foot. Tarsal tunnel syndrome is a condition that you get when this nerve is squeezed by surrounding bone, muscle, tendons or ligaments.

What Causes Tarsal Tunnel Syndrome?

The tibial nerve in the ankle/foot can be squeezed in a few different ways:

  • By swelling caused by an ankle sprain,
  • Any soft tissue swelling inside or outside of the tunnel caused by other injuries - like a ganglion cyst, swollen tendon or bone spur,
  • Degeneration (wearing away of bone and/or tissue) in the ankle joint from conditions like arthritis,
  • Any nerve damage that's already existing in your ankle,
  • Or even anatomical, natural shape and size of your tarsal tunnel or other naturally occuring factors like if you have flat feel or fallen arches.

Tarsal tunnel syndrome is often referred to by many other medial names and may be caused by some similar conditions. Here are some of the more common names or conditions that relate to tarsal tunnel syndrome:

Tendonitis (sometimes spelled tendinitis and pronounced 'tendinitus') is a condition where your tendon is irritated or inflammed. Usually this irritation, swelling and inflammation happens because of some kind of damage or micro-tearing to the tendon. This can happen from overuse of the tendon or an accident of some kind.

Foot and ankle tendonitis (posterior tibial tendonitis) can cause tarsal tunnel syndrome, especially if the tendon affected runs inside or outside of the tarsal tunnel. If this happens, your tendonitis (swelling and inflammation) will squeeze the tibial nerve in your tarsal tunnel and trigger your TTS injury.

What's the Difference Between Tendonitis, Tendinosis and Tenosynovitis?

Tendinosis is basically like tendonitis but it's not caused by an injury. Tendinosis is caused by chronic degeneration (wearing away) of tendon fibers over time. This is a natural thing that happens to our tissue as we age.

Where tendonitis is damage or micro-tearing to your tendon, tenosynovitis is damage or micro-tearing to the sheath covering your tendon. This sheath is a protective lining for your tendon and is made up of synovium. If this sheath is injured it can also swell up to take up space in your carpal tunnel and squeeze on the median nerve.

It's possible for you to have tendonitis / tendinosis AND tenosynovitis at the same time creating a more complicated wrist injury.

What is a Sprained Ankle?
How Does a Sprained Ankle Relate to TTS?

A sprained ankle is a stretch and/or tear that's happened to 1 or more of the ligaments in your ankle. You might end up with a sprained ankle if your foot twists, rolls or turns inward beyond its normal range of motion. You can easily sprain your ankle if your foot is planted unevenly on a surface.

Approximately 25,000 people suffer from the pain of a sprained ankle every day (reference: 1). There are 7 different kinds of ankle sprains that you can get including a high ankle sprain, lateral sprain, invesion sprain, eversion sprain, syndesmosis sprain, anterior talofibular ligament sprain or even an anterior inferior tibiofibular ligament sprain.

Swelling and inflammation from an ankle sprain can also cause tarsal tunnel syndrome by compressing the tibial nerve in the tarsal tunnel. Relief of swelling and inflammation through cold compression and conservative treatment will release the pressure on the tibial nerve.

How Severe is Your Sprained Ankle Injury?

Sprains are usually categorized according to severity:

  • MILD (Grade 1) - 1 or more ligaments in your ankle are stretched in a mild sprain, but there is no joint loosening.
  • MODERATE (Grade 2) - 1 or more ligaments in your ankle are partially torn, producing joint instability and some swelling.
  • SEVERE (Grade 3) - 1 or more ligaments in your ankle are torn completely, separated from the bone and causing you excruciating pain. This is a complete rupture of the ligament(s) and makes your ankle joint nonfunctional.

It's also possible to rupture a tendon in your ankle due to tendonitis or a traumatic accident.

You should seek medical attention if you suspect that you've ruptured a tendon or ligament in your ankle, can't move the affected joint and have numbness in any part of your ankle.

The good news is that most cases of tarsal tunnel syndrome will heal with simple home conservative treatments and surgery is often not needed!

But I asked the one who fitted me with this brace how long he thought I would need it, and he plainly said, "The rest of your life!" I yelled, "NOOOOOOOOOO! I will not!!! I WILL BE HEALED!" However, I didn't know how. I then began researching on the internet some other means of healing this type of injury. I found out that tendons do not heal well by themselves, so all I was doing was propping up my injury to a lifestyle of pain and hardship. By the grace of God, I had acquired a motorized chair (I called it my "Chariot") to use, which was a life-saver for about 9 months, or I would have been virtually apartment-bound in this retirement complex where we now live. So I was grateful for the chariot, but it wasn't helping my injured foot to heal. I thought it surely MUST BE healing, but that brace-fitter gave me a rude awakening that it was not, nor would it ever be! Wow! So I researched on the internet any possible alternatives for healing my foot. The MENDMESHOP had the most promising way of healing torn tendons, or tendonitis, that I could find. However, we had already spent lots of money on braces, etc., so I didn't purchase it right away. But that allowed me to see that truly, I was NOT getting any better, even in that fancy, molded-to-my foot-and-calf brace. So after more careful research and printing out the articles in your website to study, my husband and I agreed to take the plunge and order the products. Right off the bat, I found that this was a company that was honest in its dealings, because your sales rep/counselor said it had been too long since my original injury for the Freezie Wrap to be effective, and I only needed the Inferno Wrap. That was a big PLUS in my opinion, which indicated to me that MENDMESHOP was not just out to SELL if I would buy. I even argued somewhat with the rep, but he stood firm, saying I didn't need the Freezie at this point! So I followed his advice and he was so right! I received my Inferno Wrap on July 14 and began using it right away. I used the Inferno Wrap at Medium intensity, at about 2 PM and 11 PM. I followed that routine faithfully for about 10 days, and then increased the procedure to 3 times per day for about 5 more days. On August 2 (only 2 weeks and 5 days into the program) I became truly healed! TRULY HEALED, can you imagine?!? I could hardly believe it, but it was so! There was no pain even without my brace on. And I have not worn the brace since! For a couple of days I still used my "Chariot" for the longer walks I had to do, but then I began leaving it behind and walking normally -- no brace, no pain, no problem! I've even been shopping now without having to use one of those motorized chairs to get around. Thank you SO MUCH!!! I hope you will use this letter to encourage others like me to "take the plunge" and order the equipment and do the routine. It works, and I'd be happy to help anyone do it. In fact, yesterday as I was shopping, I met a woman who had a "boot" on just like the one I first had.
